Name to Appear on Check: LHES Foundation

Project Name: Lanai Community Health Inititive

Rationale for Project: In 2020 the LHES foundation was awarded $16000 from MKCF to continue our health pathways program for high school students at LHES. Due to the COVID-19 pandemic, program activities that inlcuded mentorship opportunities and a college tour trip had been stopped, but I am proud to announce that the program has restarted and has evolved.

Our program that was started in 2016 was funded through the 21 Century Learning Center grant as well as MKCF. Since then our
21 Century grant cycle has since run out. But through partnerships we created with JABSOM, they has taken our program model, received funding from Pulama Lanai and now we have 2nd year medical students who live 2 months out of the year on Lanai (January and Febrary). This year we have several exciting opportunities; a health fair school wide, a community health fair, mentorship opportunities for interested students and a college trip that includes a tour of JABSOM.

Total Amount of Funding Request: $20,000

Date Funds Will Be Needed: March 1, 2023

Itemized Breakdown of Budget Request (can be emailed separately if needed): will send an email

Please include a current budget for the proposed project (can be emailed separately if needed): will send email

Identify who would benefit from funding request (age groups, number and demographics of targeted population): Students who attend LHES, grades 9-12 who are interested in a health profession.

What specific goals are you trying to achieve? : Work with community partners to give LHES and JABSOM students exposure to rural health.

Give LHES students the opportunity to learn medical skills.

To create connections with students and our community to understand the pathway plan to getting a degree in health care.

To have a healthier Lanai in the future with doctors who are deeply connected to our island and our needs.

How will you evaluate success?: I aways evaluate success with student feedback. Over the past 7-8 year students beg me to take them on this college tour trip. I believe that the students want experiences that give them first hand knowledge on how start creating their post high school plan. They also can learn more when these trips are not combined with having to play sports.

During this trip I have students act, dress and speak in a professional manner so that they understand what it takes to be competitive in the outside world. It makes me so happy when we have Lanai college fairs and they automatically ask me if they have to be "business attire", because I now they learned something very important besides the actual pathway/content.

The evaluation is unconventional but it is based on direct feedback.

Other sources of funding the group has received or is planning on applying for: We have some funding that was donated to the LHES Foundation, Pulama Lanai is the main funders for the grant received by JABSOM.
